Use of Antimicrobial Silver in Medical Implants

Antimicrobial silver is being used in more and more medical implants – such as in catheters, shunts, and bone plates and screws – to help prevent post-surgical infection of the implant site. 

But is this practice safe?  And does it result in a significant reduction of post-surgical infection?

The good news is that based on existing clinical data, silver-coated implants are not only safe, but they do indeed dramatically reduce post-surgical infection. 

The bad news is that researchers have yet to figure out a way to more accurately control the release of silver ions from the silver-coated implants.  This means there can be times when not enough silver ions to control infection are being released, and times when more than enough ionic silver is being released from the implant. 

Researchers are now looking at ways to more carefully control the amount of silver ions being released from implants, so as to more effectively control the possibility of infection.  Here’s the story…

According to this website, silver is now used as an antimicrobial agent in numerous implant technologies, including:
  • Neuro-surgery shunts
  • Urology catheters
  • Orthopaedic implants
  • Suturing material
  • Bone plates and screws 
Even bone cement is being mixed with silver to help prevent infections (see clinical study overview, here) after bone surgery.   

What’s more, titanium plates coated with silver nanoparticles are now being used in a variety of implants, and clinical studies have demonstrated silver to be completely compatible with the body’s cells, causing no cytotoxicity while dramatically reducing levels of pathogenic microorganisms at the implant site.  (See clinical study overview, here.) 

The idea being that implants coated with antimicrobial silver will have far less chance of becoming repositories for colonies of infection microbes such as those that can cause MRSA or other serious internal infections.

At present, at least 100,000 cases of internal infection associated with medical implants – such as hip or knee joint replacements – are reported every year, at a cost to the U.S. health care system of over $1.5 billion annually. 

But a simple coating of silver on the medical implant can help keep microbial contamination of the implant site to a bare minimum, resulting in far fewer serious infections after implant surgery. 

This helps prevent the need for drastic measures should a surgical implant site become infected. 

Without silver-coated implants, the only options are long-term therapy with antibiotic drugs… surgical removal of the implant…debridement of the infected tissue surrounding the implant…and implant replacement – all of which carry serious dangers of their own. 

More Advances Coming

But while silver-coated medical implants are proving to be extremely helpful in preventing infection of the implant surgical site, they’re not perfect. 

That’s because infection prevention is completely dependent upon the release of silver ions from the silver-coated implant.  And that release of silver ions can be inconsistent, depending upon the location of the implant, the person’s body chemistry and a host of other factors. 

For example, studies have demonstrated that more silver ions are released naturally in the presence of acidic bodily fluids than in the presence of alkaline bodily fluids.  So shifts in the acidity or alkalinity of bodily fluids can increase (or decrease) the number of infection-fighting silver ions being released from a silver-coated implant.

That’s why researchers such as Rohan Shirwaiker, Assistant Professor of Industrial and Systems Engineering at North Carolina State University and others like him are constantly looking for ways to utilize silver more effectively in medical implant technology.

For example, according to this 2015 news article titled “Silver Shines as Antibacterial for Medical Implants,” Professor Shirwaiker reveals that he’s working on “engineering ways to apply a low-intensity electrical charge to a silver-titanium orthopedic implant.” 

This technique would allow a silver-coated titanium implant -- such as a hip or knee joint replacement -- to release on a constant and controlled basis “silver ions that kill or neutralize bacteria on and around the implant.”   

In other words, it’s often not enough that a titanium hip or knee joint replacement is coated with antimicrobial silver in order to help prevent microbes from colonizing in or around the implant and creating pockets of infection. 

There has to be a way to make sure the silver coating on the implant releases enough silver ions to consistently get the job of killing pathogens done, not just at the implant site, but in the surrounding tissues as well. 

After all, anytime silver is used on a medical implant, it’s going to release silver ions as soon as acidic bodily fluids react with the silver coating on the implant. 

But this natural release of silver ions from the silver-coated implant cannot be effectively controlled.  Sometimes the release of silver ions is high enough to kill all of the pathogens in the environment around the implant, and sometimes it’s not, depending upon a variety of biological factors. 

However, as Professor Shirwaiker’s research is demonstrating, if a very tiny electrical charge – perhaps battery driven, similar to the way a pacemaker works -- is applied to the silver-coated implant, then the release of silver ions can be more effectively controlled. 

The challenge, of course, is to make sure that the tiny electrical charge on the silver-coated implant does not release too much ionic silver into the surrounding area, resulting over time in silver buildup or even potential toxicity to nearby healthy cells. 

As Professor Shirwaiker points out, “One of the engineering challenges is to precisely control the level of silver that is released so that no healthy cells are compromised…”

Basically, Professor Shirwaiker’s idea is that if infection begins to show at the implant site in spite of the fact that the implant has been silver-coated, then the power source delivering silver ions from the silver-coated implant could be turned up by remote control so that more silver ions are released until the threat of infection is abated.  Then it can be turned back down. 

Professor Shirwaiker looks forward to the time when such an electrically based system controlling silver ion release from implants can be incorporated into any type of surgical implant.  

The Bottom Line

The bottom line is that silver-coated implant technology is still in its early stages, and more is being learned all of the time as researchers continue to study the technology and its benefits. 

There’s no doubt that silver-treated implants work to help prevent infection at the implant site. 

The only factor researchers haven’t pinned down yet is the ability to more effectively control the release of silver ions at the implant site in order to make sure there’s enough silver present to prevent infection, while not overloading the body’s tissues with silver which theoretically could result in a case of localized argyria (i.e., tissue staining). 

What Some People Are Doing

Some people with simple medical implants, such as catheters, are taking matters into their own hands and using colloidal silver in order to prevent infections at the surgical site. 

For example, on the Testimonials page of TheSilverEdge.com website, one woman tells how she used colloidal silver to help resolve her husband’s infections in an artificial bladder.  Here’s what she wrote:

One of my favorite things in life is seeing something I’ve harped on for decades finally get noticed by researchers and put into use.

Apparently, after all testing is complete in Brazil, the new silver-embedded plastic milk cartons will be marketed throughout Europe and the U.S. “where much larger volumes of fresh milk are consumed than in Brazil.”

NASA to Use Colloidal Silver in Manned Space Flights (Again)

If there’s ever been a case of “the more things change, the more they stay the same,” it’s this one. 

Back in the 1960’s NASA quite literally invented the technology for keeping the astronaut’s drinking water safe and germ-free using ionic colloidal silver.  And NASA used the technology in early space flights for that purpose.    

NASA later switched from using a silver-based water purification system to using one based on iodine.  Meanwhile, the Russian Space Program has continued to use a silver-based water purification technology to this very day.

Now, according to an article on Bloomberg.com, NASA is getting ready to switch back to using a silver-based water purification technology.  It’s like déjà vu all over again, as the famous baseball player Yogi Berra once stated.

Here’s the story of how the U.S. space program has come back full circle to utilizing colloidal silver in space in order to disinfect the astronaut’s drinking water, and how you can take advantage of the very same kind of technology to make your own high-quality colloidal silver at home…

In 2000 I wrote an article titled Big Business Moves into Silver-Based Antimicrobial Products in a BIG Way

It later became the most widely re-reprinted article on the growing uses of antimicrobial silver in consumer goods ever written.

In that article I revealed that NASA had actually invented the technology for producing ionic silver and using it to disinfect the astronaut’s drinking water in space:

Even NASA used silver ions to maintain the water purity aboard the Space Shuttle and International Space Station. This technology was originally developed by NASA at the very beginning of manned space flights, back in the 1960s.

At that time, scientists from NASA and the Garrett Corporation Air Research Division began a joint project to develop a water purification system for the Apollo spacecraft. This unit had to be small, lightweight and consume as little power as possible.

They produced a purifier that was slightly larger than a pack of cigarettes and weighed only nine ounces. The device used electrolysis to dispense silver ions into the spacecraft's water supply, successfully killing off any bacteria.

If this device sounds familiar, it should. It can be considered the forerunner of the common low-voltage DC current colloidal silver generator in widespread use today by savvy consumers who want to produce their own high-quality colloidal silver quickly and inexpensively!

NASA's ingenuity at controlling microbial contamination in the Apollo spacecraft's water supply opened the doors for safer methods of controlling water pollutants here on Earth.

NASA held the patent on the device, but authorized a company in Cornelia, Georgia, called Carefree Clearwater, to manufacture this device for civilian use. This lead to the creation of a whole new industry based on the power of silver ions to eradicate microbial contaminants.

Since that time, numerous companies have developed their own version of NASA's silver ion generator.

In April 2009 I wrote and published the article Silver: Nature's Purifier Used on the Space Shuttle and In Swimming Pools, in which I pointed out:

“After testing 23 methods of purifying water, NASA also chose silver as the purifying agent on the Space Shuttle Program.

Silver is being used in two functions that provide Shuttle crews with pure water for drinking, air conditioning, food preparation and other operations.

Water wastes are recycled in Shuttle flights and silver's first job is to treat hydrogen-saturated water coming from the Shuttle fuel cells: this water passes through a tubular device of palladium and silver alloy.

From the silver-palladium tubes the water flows to a purifying unit where silver eliminates bacteria, including Pseudomonas A and type III A bacteria, NASA scientists report.

By establishing 100 parts of silver in a billion parts of water as hygenic for drinking in the Shuttle, NASA eliminates the need for 1,000 to 1,500 parts per billion of chlorine generally used for purification.

The unit currently used provides Shuttle crews with 32 gallons of pure water daily for all uses within the Shuttle, as well as for backpacks when the astronauts work outside the vehicle in space.

Compared to earlier prototypes, the new unit weighs 90% less, needs only one-third the space. It also doubles the production of water and simplifies the process: it eliminates the need for mixing, metering and testing water while in flight and eliminates the risk of corrosion.”

Hate Mail

Shortly afterwards, I was barraged with emails from angry individuals claiming that silver has never been used on the Space Shuttle, or in any manned U.S. space missions, and that the water purification method of choice for U.S. manned space flights is iodine.

This, of course, was only partially true: 

The U.S. had indeed switched to the use of iodine for water purification inside the International Space Station, while the Russians – who control one-half of the space station – stood firm with the original silver-based technology to keep the water their cosmonauts drink fresh, pure and germ-free.

So silver was (and is) still being used in space missions, but by the Russians and not the Americans. 

When the barrage of hate emails claiming that silver had never been used by Americans in space finally began to ebb, I posted a document I found on the NASA.gov website, demonstrating that colloidal silver had indeed been used in space flights by the U.S. since the mid-1960’s, and that NASA had invented the technology.

I also wrote and published the articleHave NASA or the Russian Space Program Ever Used Colloidal Silver as a Water Disinfectant on Manned Space Flights?”, in which I pointed out that way back in 1976 when the Space Shuttle first began missions with astronauts on board, silver was used to disinfect the onboard water. 

Indeed, I uncovered a 1976 press release from The Silver Institute which stated, “Silver will be used in two functions that will provide Shuttle crews with pure water for drinking, air conditioning, food preparation and other operations.”

And going back even further, I revealed a May 1973 press release from The Silver Institute explaining that the Russians, too, were using silver-based water purification systems in space:

Russian scientists working on water recycling and purification problems for the Soviet space ship and orbiting station program have decided on silver as the best long-term sanitizing agent. 

Researching the problems of water storage over periods of several months, as well as purification for immediate use, they determined that ionized silver provides the safest and longest lasting method of transforming polluted waste in potable water.

A significant fact in support of their decision to use silver for purification was their experimental confirmation of the absence of toxicity in the silver treated water.

In lengthy experiments on animals they found that 100 parts and 200 parts of silver per billion in drinking water does not accumulate in the organism and does not produce any detrimental effect on the functioning of the organs or systems of the experimental animals.

This was also confirmed by year long experiments on volunteer human subjects.
The scientists, Drs. S. V. Chizhov, S. P. Pak, N. N. Sitnikova and Y.U. Koloskova tried many methods of purifying regenerated water but all except the silver system proved unsatisfactory over the long run.

In sum, the Russians found silver to be the safest sterilizing agent, stable, and long-lasting.” 

Déjà vu All Over Again

Now, according to an August 25, 2015 article on Bloomberg.com, antimicrobial silver is apparently making a come-back in the American space program. 

Bloomberg states:

The space station carries roughly 2,000 liters of water in reserve for emergencies, split about evenly between the U.S. and the Russian sections of the International Space Station.

The two sides operate separate water systems mainly because of decades-old decisions on how best to disinfect water…

…the U.S. relies on iodine and the Russians relying on a different go-to:  silver, which in its ionic form is a powerful antimicrobial agent…

…NASA has decided to switch to silver-ionized water on future missions..."

So there you have it.  We’ve come back full circle.  According to Bloomberg, future NASA space missions will utilize antimicrobial silver for water disinfection purposes.

Silver Coated Wall Panels for Hospitals Stop MRSA

Knowing that hospital-acquired MRSA infections kill between 15,000 and 19,000 patients a year, wouldn’t you feel safer going into a hospital that uses wall and ceiling panels coated with MRSA-killing antimicrobial silver nanoparticles?

According to the June 2015 issue of Silver News, a German company called Lamilux has invented wall panels coated with silver nanoparticles. 

The fiber-reinforced plastic wall panels can easily be installed on the walls of hospital operating rooms, similar to the way tiles are laid on a floor. 

The goal is to help prevent MRSA growth and other forms of bacterial contamination, which are common in operating rooms and which infect up to 94,000 hospital patients every year.  News sources claim approximately 20% of the MRSA infections that go blood-borne result in the deaths of the patients, meaning these new panels have the potential to help stop approximately 18,000 hospital-caused deaths every year.  

Clinical Trials in Germany

According to the Silver News, the new panels (i.e., wall tiles) are now undergoing clinical trials in Germany to see how effective they are.  They wrote:

Lamilux Composites GmbH of Rehau, Germany, is offering a fiber-reinforced plastic material with a nanosilver coating that company officials say mitigates germs within a few hours.

Initially developed as a wall coating for hospital operating rooms, the company’s AntiBac product was developed with Rent-a-Scientist, a Regensburg-based company that specializes in nanosilver research.

With the rise of multi-resistant germs in hospitals, sometimes lethal to patients, Lamilux officials note that one in particular — methycillin-resistant Staphylococcus aureus or MRSA — appears regularly in operating rooms and can cause wound infections and respiratory inflammation that can lead to death. MRSA is resistant to many drugs.

In addition to clinical use, AntiBac can be used in the food industry in refrigerated warehouses, cold stores and refrigerated vehicles as well as in processing areas and slaughter houses, according to company officials.

The new composite is being tested at the Asklepios Clinic in Bad Abbach, Germany, where some parts of operating room walls have been covered with the anti-bacterial panels.”

The Lamilux company website claims the silver-coated panels represent, “…a new technologically-advanced composite material: a fibre-reinforced plastic, on the surface of which a nanosilver coating gives it an antibacterial effect and kills germs within just a few hours.”

Lamilux goes on to explain the reason for the development of the new silver-coated panels:

Multi-resistant germs pose a major threat in hospitals and could be fatal for patients.

Clinics therefore permanently face the challenge of preventing the occurrence of dangerous hospital germs with targeted preventive and emergency hygiene measures.

…The new antibacterial LAMILUX material can in the future be used as wall and ceiling panels…it has been scientifically proven that dangerous germs die within a few hours.

In addition to clinical use, there are also numerous areas of application for the high-tech material, mainly in the food industry, where high hygiene standards need to be maintained and sterility is paramount.

…The material is being trialed in practice at the Asklepios Clinic in Bad Abbach, Germany. In some parts of the operating room, the walls have been covered with the anti-infective panels.”

Of course, this is not the first time antimicrobial silver has been employed to help staunch the epidemic of hospital-acquired MRSA infections. 

According to the Colloidal Silver Cures MRSA website, “…hospitals in Britain have been testing pajamas and bed linen made of a special fabric that is impregnated with silver, as a means of stopping the spread of MRSA dead in its tracks…Over the course of the next year, the specially created fabrics are being used at the Lister Hospital in the city of Stevenage, in Hertfordshire, England.

It is already used in plasters and hospital dressings. But now medics from Leicester University have created a generator that divides pieces of silver into billions of tiny particles.

It then suspends them in liquid glycerol to be put into aerosols. The particles are small enough to pass inside bacteria but do not kill human cells.

The $250,000 machine’s inventor, Professor Chris Binns, said medical trials will start within the year. He also believes the silver ‘bullets’ produced by his machine could be used to fight HIV.”
